Volunteer Coaches
Information for all Divisions
Volunteer coaches make the River Forest Park District youth
programs possible. We are currently in need of coaches, both
beginner and experienced, for our recreational league. All
volunteers must fill out a volunteer wavier and criminal
background check consent form. Upon receipt of your registration
form or phone call, we will send a letter of confirmation
detailing the date and time of the coaches orientation. If
you’re not sure about head coaching a team, co-coaching
or assistant coaching can be arranged. Volunteering is fun
and rewarding! The District is grateful to those of you who
have donated your time in the past. Thanks!

grade you would like to coach.

Volunteer Requirements
Coaches Orientation *MANDATORY*
Come join us for pizza and pop, followed by the orientation
which will address our safety policy, accident/incident reporting,
emergency procedures, program rules of conduct, and mandated
coach’s forms. Coaches who pre-register will be sent
the mandatory forms before the orientation and must be returned
at the Coaches Orientation. (You may also download the printable forms below, fill them out and bring them to the Coaches Orientation).
Criminal Background Waivers -- [Download Form Here]
The River Forest Park District has adopted a procedure to better
safeguard the children and youth in our programs. All new
full-time employees, part-time and seasonal instructors,
coaches and volunteers working in child-sensitive positions
are subject to a criminal background investigation. This
procedure requires checks of potential staff and volunteers
for criminal convictions relating to inappropriate behavior.
Each season, background investigations will be conducted prior to the beginning of the sport or activity in which you will be involved, unless you volunteer on a continuous, year-round basis. We appreciate your understanding that the safety of our children and youth are our top priority.

The Park District requests this information on coaches for
their own safety. A copy of the information is kept at Park
District Headquarters. All information is kept private and
will be accessed only in case of an emergency.
Volunteer Waiver & Release -- [Download Form Here]
The Park District requests all volunteers to fill out the
volunteer Waiver & Release. Please read the form carefully
and be aware that in signing up and volunteering for coaching,
you will be expressly assuming the risk and legal liability
and waiving and releasing all claims for injuries, damages
or loss which you may sustain as a result of participating
in any and all activities connected with and associated with
your volunteer services (including transportation services,
when provided).
